The affidavit is required in connection with approval of a request for a Variance of side yard setback requirements for construction of a residence addition, and for location of a spa and spa equipment in the side yard of Lot 31-RH, per Zoning Case No. 344. Enclosed also is a copy of the Findings and Report, prepared pursuant to the action by the Planning Commission. The Findings and Report and the recorded Affidavit of Acceptance form are to be retained in your file on the property. The Council voted to ratify the Planning Commission's approval of the request on October 20, 1987. A copy of the Findings and Report for Zoning Case No. 344 is enclosed for your information. An acceptance form, as required by Ordinance No. 207, is also enclosed. Please complete the form, have the signature(s) notarized, and forward the document to the Office of the County Recorder, Room 15, 227 North .Broadway, Los Angeles, CA 90012 with a check in the amount of $7.00. When the Affidavit of Acceptance has been returned to the City, duly executed and recorded, the Los Angeles County Department of Building and Safety will be notifed that a permit can be issued. A copy of the minutes is enclosed for your information. Thy Planning Commission's decision will be reported to the City Cornell, as required by the Municipal Code, Section 17.32.090, at their next regular meeting on Monday, November 9, 1987, pursuant to the requirements of the Municipal.Code, Section 17.32.140.
