Pursuant to Section 17.32.087, Ordinance No. 207, an Affidavit of Acceptance form must be executed before the above approval becomes effective. A copy of the Resolution of Approval, specifying conditions of approval set forth by the Planning Commission, is enclosed for your information. Once you have reviewed the Resolution of Approval, please complete the enclosed Affidavit of Acceptance form, have the signature(s) notarized, and forward the Affidavit to the Office of the County Recorder, Room 15, 227 North Broadway, Los Angeles, CA 90012, with a check in the amount of $ 9.00. We have obtained a new survey and it shows that the corner of the old house is 9.7 feet from the property line and that the new addition is 6.7 feet from the property line. A copy is enclosed. The forms are in for the new foundation and $6,800 worth of custom run tile along with all the framing for framing is on site. All of the necessary approvals and permits have been obtained from the city and county. Rolling Hills, Ca. 90274 Dear Mr. Hamada: This is a request for the City of Rolling Hills to approve a modification of the plans which were approved several months ago for our addition at 18 Chuckwagon Rd. All permits were issued previously; but at the time of pouring the foundation, the county inspector requested a side yard survey which, he said should have been ordered before the permits were issued. A survey revealed the original structure was not placed on the lot in accordance with the plans on file with the city. I am enclosing a new plot plan from Russ Barto, AID, the architect of the addition and a copy of the survey. A legal description of where the new addition is placed in the easement is being prepared by the survey company. I believe you have already received a copy of a letter from Peggy Minor telling us of the Association's decision to grant us a License Agreement to use a portion of the easement.
