Keep a copy for your records and forward the Acceptance Form and

Los Angeles, CA 90012 with a check in the amount of S 5.00 for the
first page and $ 3.00 for each additional page.
The City will notify the Los Angeles County Building & Safety
Division to issue permits only when the recorded Affidavit of

A preliminary review of the application noted above has been
completed by the City's staff and pursuant to state law wefind
that the information submitted is:
X Not Complete. The application has been held in abeyance
because certain information is missing, you failed to comply
with certain requirements, or both. The information needed
to complete the application is listed below, and must be
supplied before the application can be deemed complete.
Additional Information/Requirements:
1. Property owner's radius map and property owner's labels. A map
is required showing the existing properties, within or partially
within a 1,000 foot radius- of the exterior boundaries of_ the
property under consideration, including the subject property.
Prepare two lists of property owners of each parcel with mailing
address on mailing labels. Assign a number to each name on the
list to correspond with the map numbers.
2. Certified Property Owners List Affidavit.
3. Show the 50 foot setback from the front easement line, 20 foot
side setbacks, and the 50 foot setback from the rear property line
on the plot plan.
4. Denote, the square footage separately of the breakfast nook and
the bay window.
5. Prepare 15 sets of the proposed project FOLDED to approximately
8 1/2" x 11" with title block visible.
